The Medical Network One's Michigan Institute for Health Enhancement medically-based disease management program is designed to coach patients in controlling their chronic illness through lifestyle modification.

The program is open to individuals and maybe covered by your insurance if you have the following condition conditions but not limited to:

  • Persistent asthma
  • Chronic obstructive pulmonary disease
  • Diabetes mellitus
  • Chronic pain syndromes
  • Coronary heart disease
  • Congestive heart failure

This program consists of three main components: nutrition, exercise and wellness coaching. The clinical staff includes Registered Nurses, Registered Dietitians, Certified Diabetes Educators, Certified Exercise Specialists, and Wellness Coaches.

  • The nutrition portion of the program includes individualized nutrition assessments, individualized meal plans, personal goal setting, and group and one-on-one coaching.
  • The exercise component of the program involves exercise assessments, individualized work out plans, personal goal setting, and group or individual exercise sessions. Medical clearance from your health care provider is required to participate in the exercise portion of the program.
  • The wellness coaching component of the program includes an initial visit with a wellness coach followed by monthly coaching calls. Additional visits with coaches are available for those patients who need extra help in achieving their lifestyle management goals.

Most health plans are accepted; co-pays/co-insurance or deductible amounts may apply.
