What is Care Management?
Care Management refers to a set of activities that assist patients and their support systems to manage medical and psychosocial problems with the aim of improving healthcare and reducing the need for costly and often redundant medical services. Care managers operate in the primary care setting or the patient’s home to provide interventions for patients with a mild to moderate chronic disease as well as high complexity, high cost patients who frequently utilize the healthcare system.
MedNetOne Health Solutions is actively involved with the Michigan Primary Care Transformation Project (MiPCT), a multi-payer, multi-year CMS demonstration project aimed at improving health in the state, making care more affordable, and strengthening the patient-care team relationship. MiPCT is state-wide in scope and is the largest Patient-Centered Medical Home (PCMH) project in the nation. Five hundred primary care practices and 1,800 primary care physicians that are affiliated with one of 41 physician/physician hospital organizations (POs/PHOs) are eligible to receive payments.
