Care Coordination Training

Several years ago Medical Network One introduced a multidisciplinary care management strategy to its physician members. Some primary care physicians welcomed the opportunity to include an RN, behavioral health specialist, certified diabetes edicator, or a dietitian as a member of the practice team while others were not enthusiastic about the concept. Perhaps this new opportunity made available by BCBSM will lead to an immediate change in your care management strategy.
BCBSM recently announced that Medical Assistants and Community Health Workers can now provide certain care management and care coordination services which are billable; however, specific planning, training and documentation requirements must be met. Program metrics, measuring success have been identified by BCBSM. They include improved outcomes such as lower emergency department use, fewer inpatient stays and consistent delivery of recommended services, such as cancer screening, hypertension and diabetes management plus the number of patients engaged in care management activities.
In order to engage your Medical Assistants and Community Health Workers in care management activitieswhich will be billable they must complete specific training and complete a post-training exam. Please refer to the attached flier for training dates and training locations. There is no cost for this learning opportunity but slots are filling up quickly.
